Digital innovation for improving survivorship care delivery

WP9 prepares the implementation of digital tools for remote patient monitoring, self-management and supportive care delivery during survivorship phase at EU MSs to improve comprehensive survivorship care delivery, patient empowerment and quality of life. WP9 specific objectives are to map existing digital solutions for survivorship care, define minimum requirements and understand reimbursement policies across EU MSs. To build an implementation package for integrating digital solutions for survivorship care in precision medicine centres. And to prepare for the integration of digital solutions to provide comprehensive follow-up and survivorship care.

Tasks

Task 9.1: Mapping of existing digital solutions for remote patient monitoring, patient self-management, and supportive care delivery in European Union Member States/in European Union Comprehensive Cancer Centres

(Subtask 9.1.1) Mapping existing digital tools for patient monitoring, personalised self-management and supportive care delivery during post-treatment survivorship phase with systematic review & survey. This will feed the a pilot study ‘Digital tools for survivorship care delivery (WP9 Pilot)'.

(Subtask 9.1.2) Report on minimum quality requirements for digital tools targeting comprehensive survivorship care needs, leveraging Subtask 9.1.1 and expertise of partners.

Task 9.2: Creation of implementation package by sharing expertise and capacity building on digital solutions for remote patient monitoring, patient self-management, and supportive care delivery in European Union Member States

(Subtask 9.2.1): Conduct literature reviews, surveys, and mapping exercises to identify/collect experience on key aspects for successful implementation of digital tools for survivorship care delivery.

(Subtask 9.2.2): Prepare the content of education and training programs on implementing digital tools for survivorship care delivery.

(Subtask 9.2.3): Conduct literature review, surveys, and mapping exercises to identify/collect experience on initiatives to foster digital inclusion among patients with lower digital literacy

Task 9.3: Preparing for the integration of additional data dimensions or solutions into digitally enabled pathways for patient monitoring, self-management, and supportive care delivery during survivorship phase

Prepare the future digital solutions, integrating data dimensions and digital infrastructures that will likely become available for routine care in the upcoming years.

(Subtask 9.3.1): Develop a framework for integrating data of health behavior into existing remote monitoring and self-management tools during the survivorship phase.

(Subtask 9.3.2): Develop a framework to include biomarkers of disease relapse into a digital survivorship care plan.
